Next Generation Muslim Education Hub Inc.

This Parent Waiver & Release of Liability Agreement (“Agreement”) is entered into by and between Next Generation Muslim Education Hub Inc. (NGMEH) and the undersigned parent or legal guardian(“Parent/Guardian”) of the enrolled student (“Participant”).

By signing this Agreement, the Parent/Guardian acknowledges and agrees to the following terms related to the Participant’s enrollment in NGMEH programs, including but not limited to STEM and Robotics programs, academic courses, and educational services (the “Program”).

1. Assumption of Risk

I, as the Parent/Guardian, understand that:

  • The Program involves hands-on activities, including robotics, coding, electronics, and mechanical work, which may pose minor risks such as cuts, strains, allergic reactions, or accidental falls.
  • The Participant will use computers, software, internet-based learning tools, robotics kits, and electrical components, and while NGMEH ensures safety guidelines, accidents may still occur.
  • If the Program includes in-person events, field trips, or competitions, there may be risks associated with travel and group activities.
  • I voluntarily assume all risks associated with my child’s participation in the Program.

2. Medical Authorization & Emergency Treatment

  • I certify that my child is physically and mentally fit to participate in the Program.
  • In the event of a medical emergency, I authorize NGMEH staff to seek emergency medical treatment for my child.
  • I understand that NGMEH does not provide medical insurance and that I am responsible for any medical costs incurred.

Parents are required to:

  • List any known allergies, medical conditions, or special accommodations required for your child.
  • Emergency Contact Name & Phone Number.

3. Release of Liability & Indemnification

I, the Parent/Guardian, hereby waive and release NGMEH, its staff, instructors, volunteers, and partners from any liability for:

  • Personal injury, illness, or accidents that may occur during Program activities.
  • Damage or loss of personal property brought to in-person sessions or events.
  • Internet security risks for online learning sessions, including cyber threats beyond NGMEH’s control.

I agree to indemnify and hold NGMEH harmless from any claims, losses, or damages arising from my child’s participation in the Program.

4. Behavior & Code of Conduct

  • I understand that my child must follow NGMEH’s Code of Conduct, which requires respect, safety, and appropriate behavior.
  • If my child violates the Code of Conduct, NGMEH reserves the right to suspend or remove them from the Program without refund.

5. Media & Photography Consent

I grant NGMEH permission to photograph, record, and use my child’s image, voice, and name for:

  • Promotional materials (brochures, social media, website, newsletters).
  • Educational documentation (learning reports, presentations).
  • I understand that no personal or identifying information will be disclosed without consent.

6. Internet & Technology Usage Agreement

  • I acknowledge that my child may use NGMEH’s online learning tools, coding platforms, and internet-based educational resources.
  • I understand that while NGMEH takes measures to protect children online, I am responsible for monitoring my child’s internet use outside program hours.

7. Program Withdrawal & Refund Policy

  • If I choose to withdraw my child from the Program, I understand that refunds are only available if requested at least [X days] before the start date.
  • If my child is removed due to a Code of Conduct violation, no refund will be issued.

8. Learning Center Partner Waiver (For Third-Party Facilities)

If my child is attending the Program at a partner learning center, I acknowledge that:

  • NGMEH is not responsible for learning center facilities, staff, or policies.
  • NGMEH will ensure quality education, but learning centers retain liability for on-site safety and supervision.
  • Any issues with facility conditions, non-NGMEH staff behavior, or additional fees must be addressed directly with the learning center.

9. Governing Law & Dispute Resolution

  • This Agreement is governed by the laws of Ontario, Canada.
  • Any disputes will be resolved through mediation and arbitration before legal action is considered.
